When hiring a new employee, use the employee app above. Be sure to complete it within the time allowed for your waiting period. If you do not remember your waiting period, please contact us at (800) 967-6205. After completing, it should be faxed to us at (260) 248-2558. The form below is used when there is a change in status for an existing employee, such as new child, new spouse, divorce, etc. It should also be faxed to us.
